PARTICIPANT REVOCATION FORM

If you have decided that you want to withdraw the information that your questionnaire response has provided for this project, you can request that the data is removed and erased using this form.

Please use the Questionnaire Response Identifier (QRI) that was provided in the Questionnaire Acknowledgement following the original submission of your response to my questionnaire, and then click 'Remove my data'.

Your request for data removal will be sent anonymously and upon receipt, the data that you provided earlier will immediately be removed from the project and erased.
